This form must be completed by all those with parental responsibility for a child in order to register your child for entry to St Piran's School.

Please note: registration of your child as a prospective pupil does not secure your child a place at the School but does ensure that your child will be considered for selection as a pupil at the School. A non-refundable registration fee of £120 must be paid at the time of submission of this form.

DECLARATION We (as the holders of parental responsibility for him/her) request that the name of the above-named child be registered as a prospective pupil of the School AND we have made the payment for the non-refundable Registration Fee of £120. By signing this Registration Form we understand and accept that: 1. registration of our child as a prospective pupil does not secure our child a place at the School but does ensure that our child will be considered for selection as a pupil at the School. 2. if our child is offered a place at the School, such an offer will be subject to the School's terms and conditions for the provision of educational services(in accordance with the Parent Contract Terms and Conditions), which will bind us (as the holders of parental responsibility for him/her) in the event (and from the moment) that we accept the place; (A copy of the current Parent Contract Terms and Conditions is available for your information on the School website or upon request from the Registrar at any time, but please note that the version of the parent contract supplied may be subject to change prior to the point in time when a place at the School for your child may be offered.) 3. if our child is offered a place at the School, such an offer will be subject to us confirming that our child has the right to enter, live and study in the United Kingdom. 4. if applicable, the School may request from our child's present school or educational institution: (a) information and a reference in respect of our child; and/or (b) information about any outstanding fees and/or supplemental charges. 5. The School, as a registered Data Controller, will process personal data about us and our child/children, including sensitive personal data such as medical details and consent to the processing of our data and our child’s data. (For further information on how we use your data please refer to the Privacy Notice and Data Protection Policy on the School Website and which is otherwise available from the School at any time upon request) Even if your child is not offered a place at the School, or you do not accept an offer, we retain information about prospective pupils and their parents for a year after the admissions process subject to your rights under data protection law. *
